Social Security Number Verification / Identity Verification and Address History This search can determine the following crucial information (a) whether the Social Security Number (SSN) is validly issued (b) to whom does the SSN belong (c) in which year and state the SSN was issued (d) the current and other known or previous addresses to that SSN. The SSN verification process can reveal that the number belongs to another individual, has more than one name associated therewith, produces other aliases, including maiden and divorced names, is associated with fraud, is not a validly issued SSN, belongs to a deceased person, produces additional addresses not stated by the application/request form, or has other SSNs associated with that individual. Criminal Record Searches Federal Criminal District Records SearchThis search is conducted through the federal district court and may include charges such as bank robbery, embezzlement, tax evasion, mail fraud, or crimes occurring across state lines. Statewide Criminal Records SearchPhysician Screening Services will conduct a statewide records search for felonies and misdemeanors. County Criminal Records SearchPhysician Screening Services will investigate any county criminal record to uncover felony and misdemeanor convictions. Examples of criminal records include theft, assault, robbery, and drug possession. Motor Vehicle ReportThe driving record may provide identifying information such as a full name and physical description, as well as dates of convictions, violations and accidents, sections violated, docket numbers, court locations, and accident report numbers. Fraud and Abuse Control Information System (FACIS®)FACIS includes over 450,000 adverse records of debarred, disciplined, excluded or sanctioned individuals and entities. Using this data set, Physician Screening Services can offer the most comprehensive and cost-effective search of its kind in health care. Additionally, FACIS provides cumulative and historical information about disciplined and sanctioned activity from over 800 federal and state agencies. Physician Screening Services will query the data service, verify probable hits, and include findings within a completed report. *Note: this search complies with DHHS (OIG) and GSA guidelines. FACIS® also includes the following search at no additional fee: Office of Foreign Assets Control (OFAC) Specially DesignatedNationals and Blocked Persons (SDN) Search Physician Screening Services will search the list of persons and/or entities that have been determined by the United States Secretary of State to be terrorists, support terrorism and/or to threaten the security of U.S. nationals, national security, foreign policy, or the economy of the United States. Advanced Screening ServicesPhysician Screening Services can recommend which of the services below can best meet your organization's needs: Civil Suits and Judgments and Bankruptcy Records Civil Suits and Judgments-County LevelThis search provides records pertaining to civil suits filed by or against an individual. Physician Screening Services will provide a copy of the complaint and the final order, or if a case is pending, a summary of the status of the suit. These records are obtained through county and state clerks of court. Federal Civil Records SearchIndividuals sued in federal civil court may be involved in disputes with considerations greater than $50,000, suits filed between parties residing in different states or an individual in violation of Federal law such as Title VII, the Americans with Disabilities Act or the Fair Labor Standards Act. Consumer Credit ReportAnalysis of an individual's credit history. The written history may provide information such as places of employment or prior addresses, credit account types, terms, amounts past due, loan types, balances, public records, high credit, dates opened and closed, payment patterns, credit limits, and modes of payment. Federal District Bankruptcy Records SearchThese searches are conducted by review of U.S. District Bankruptcy Courts. Findings may include Chapter 7, 11, and 13 filings. Sexual Offender RegistryPhysician Screening Services will query the name of the subject against an existing sexual offender registry at the state level.
